c7f9c1bac2 feat(agent-team): Confluence-writer node (draft -> approve gate -> write)
Add a Confluence documentation lane to the Plane-2 pipeline, flag-gated behind
AGENT_TEAM_CONFLUENCE_ENABLED (default off; daemon behavior unchanged when off).

- confluence/client.py: OAuth 2LO + Basic REST client, dry-run-default writes
- confluence/mermaid.py: vendored ADF-only Mermaid editor (macro-count +
  revert-diff guards, dry-run default)
- nodes/confluence_writer.py(+_llm): conf_draft -> conf_gate -> conf_write,
  both direct (task_kind=confluence) and post-build documentation flows
- task_model/graph/coordinator: new phases, state channels, route_after_intake,
  CONFLUENCE_APPROVAL_KIND gate delivery, task_kind forwarding
- db schema v5: widen pending_questions kind CHECK (atomic rebuild)
- tests for client, mermaid, writer node, ledger v5, coordinator gate, e2e

1b4d30e47f feat(agent-team): add pending_questions.kind discriminator + migration (Phase B1)
The plan-review gate (coming next) needs to tell its decision questions apart
from clarifier questions in the durable ledger. Add a `kind` column to
pending_questions (values 'clarify' | 'plan_decision').

- Fresh DBs: `kind TEXT NOT NULL DEFAULT 'clarify'` (+ CHECK) in the DDL.
- Live ledger: idempotent additive migration (SCHEMA_VERSION 3→4) — a guarded
  ALTER (PRAGMA table_info) run from both migrate() and init_db; legacy rows
  take the 'clarify' default, never null. (SQLite can't add a CHECK via ALTER,
  so the migrated column is NOT NULL DEFAULT only; value constraint is enforced
  on fresh DBs by the CHECK and on all writes by the typed helper.)
- ledger.post_question gains a keyword-only `kind="clarify"` (backward
  compatible — existing callers unchanged); PendingQuestion.from_row reads it.

Tests: fresh-DB column+default, idempotent init_db, legacy-DB backfill to
'clarify', plan_decision round-trip. 1396 passed.

15a416d31a Add Plane-2 leaf scaffold (pipeline graph, nodes, HITL, transports, CI)
Consolidates the 18 leaf modules from the r720-plane2-scaffold workflow onto
the foundation commit. Full suite: 535 passed, 1 skipped; ruff + format clean.

Built (pre-deployment scaffold only — nothing provisioned/enabled):
- LangGraph pipeline graph.py (INTAKE->CLARIFY->PLAN, interrupt()/resume, checkpointer-injectable)
- nodes: clarifier (98% gate), planner, review_loop (GPT-4.1), builders->candidate diff, verifier
- §3.3.1 HITL: ledger ops, resume_worker, deadline_timer, recovery sweep, responder
- transports: slack / github / claude_code adapters
- ci_gate (pure-code pass/fail), operator_cli, run-team.py entry, P1 sim harness
- ci/agent-team-apply-verify.yml (split untrusted/privileged jobs) — authored, disabled

KNOWN OPEN FINDINGS (verifier/cross-review, not yet fixed — see follow-up):
- builders denylist: 4 execution-proven bypasses (delete, mode-change, copy-to, out-of-scope delete)
- §3.3.1 CAS: BEGIN IMMEDIATE outside try/except; shared-connection txn nesting unsafe under concurrency
- operator_cli: missing re-deliver/force-resume; audit-after-mutate ordering gap
- ci yaml: GPT-4.1 cross-review PASS w/ 4 FIX items (symlink path escape, etc.)
- P1 sim harness models the ledger layer, not real LangGraph interrupt/resume; P1 exit criteria not yet truly proven

Deploy-gated (NOT done): IAM/step-ca/Roles Anywhere/confluence-bot provisioning,
/sh-security-review sign-off, live Slack/CI, rsync, live dry-runs, Adam approval.
