docs: update AUDIT-REPORT.md for Phase 5 Medium fixes

Browse source 
17 Medium findings fixed across API, Web, Lambda, and Infra:
- API: M3, M4, M6, M8, M11, M14
- Web: M2, M5, M6, M7, M10, M13
- Lambda: M1, M5, M8
- Infra: M5, M8
CI pipeline now runs all 108 tests (dotnet, vitest, pytest)
