* feat: link Slack thread/Linear ticket in PRs and append ticket to title When opening PRs, include any referenced Slack thread or Linear ticket in the description and append the resolvable ticket number to the title. Adds a Slack permalink to the webhook prompt so the agent has a ready-to-link thread URL.
