Co-authored-by: open-swe[bot] * fix: order Fable 5 after Opus 4.8 so stale-Anthropic fallback is preserved provider_fallback_pair picks the first same-provider model, so placing Fable 5 first redirected stale Opus selections to it. Keep Opus 4.8 first.
